Please complete the referral form below and upload your referral letter and any other relevant documentation including a Mental Health Care Plan. Our team will review the referral and match your patient with the most appropriate clinician based on their needs, availability and preferences.

What happens next

Once we receive the referral, our administration team will review it within two business days. We will then contact the patient directly to arrange an appointment.
